The mission of AIDS Services of Austin is to enhance the health and well-being of the community and people affected by HIV and AIDS. For more than 27 years, ASA has provided a continuum of services to the greater Central Texas area.

As a Medical Case Manager you will work with HIV positive clients as they navigate new diagnoses, medical appointments, medication, substance use, mental health symptoms, and medical comorbidities. With patience and compassion, you will guide clients, provide resources, and encourage long term stabilization and retention in medical care.

You will have the opportunity to work with a diverse population, acting as part of a multidisciplinary team, and occasionally sharing clients with LVN and Non-Medical Case Managers as dictated by client acuity. Medical Case Managers may also touch on housing, income and language needs as required.

A successful Medical Case Manager must be willing and able to:

  • Conduct comprehensive assessments of client needs on an annual basis
  • Rate clients' acuity level
  • Screen clients for mental health or substance abuse issues
  • Develop services plans along with clients
  • Track progress of client's service plans
  • Document all client interactions in internal database

You Have:

  • An LMSW licensure

  • Experience working with HIV/AIDS disease
  • Experience working with individuals of diverse cultures, ethnicities, sexual orientations, and gender identity
  • Experience working with clients with substance use disorders
  • Training in harm reduction principles and motivational interviewing skills
  • Bilingual/bicultural skills (English/Spanish)
